We routinely ask all our newly registered patients to complete an Alcohol questionnaire and Patient Information Form  and we would appreciate it if you would take a little time to complete the forms attached to your welcome email, which  you can either email back to us or print off and hand in at any of our surgeries.

If you have an existing medical condition (for example Asthma or Diabetes), please contact us to make an appointment with a GP so that we can set up your repeat prescriptions, reviews etc.
