We routinely ask all our newly registered patients to complete an Alcohol questionnaire and Patient Information Form  and we would appreciate it if you would take a little time to complete the forms attached to your welcome email, which  you can either email back to us or print off and hand in at any of our surgeries.

If you have an existing medical condition (for example Asthma or Diabetes), please contact us to make an appointment with a GP so that we can set up your repeat prescriptions, reviews etc.

Patient Access

Using Patient Access, you can view, book and cancel GP and Nurse appointments at your local GP surgery from home, work or on the move – wherever you can connect to the internet. What’s more, because Patient Access is a 24 hour online service – you can do this in your own time, day or night.

Plus, Patient Access is not just for booking appointments – the popularity of online appointment booking has led to more online features being added, including:

  • Viewing your medical record

Secure personal information

  • All information that is sent to your surgery via Patient Access is secure. Your personal details are encrypted and protected using the highest standard internet security, so it cannot be intercepted. Only you and your GP surgery are able to see this information.

You will then need to call into one of our Surgeries with two forms of identification (one must be photographic ID) and our Reception Team can activate your Patient Access Account.
