An important notice for our patients,
Following the IT clinical system change over in August 2022, Patient Access will no longer be available for Unity Health patients to use. If you are wanting to view your medical records online, please download the NHS App.
You will need a linkage key, account ID and ODS code from the surgery to view your records on the NHS App.
To obtain this information, please bring a form of photo ID into the surgery or call us on 01904 754 900 so that our reception team can ask you some security questions.

How do I register with the NHS App?
You can set up an account yourself with the NHS App – the app can verify your identity without you coming into the surgery.

What do I do if I cannot use the NHS App?
If you are unable to use the NHS App, you can contact the surgery on 01904 754900.
What do I do if I have forgotten my log-in for the NHS App?
The surgery can only give patients access to their medical records on the NHS App.
If you are having technical issues, such as forgetting your log-in, or glitches within the app, please contact the NHS App Technical Support Team.
