You can access your medical records providing that you have requested this is writing. Requests will be actioned within 21 days of receipt of your request.
Please direct your written requests to The Reception Services Manager, c/o your local surgery address.
Some types of access have charges attached to them, these charges are set nationally.

A double appointment will be made with a nurse so you can go through your records. The nurse is there is explain any technical information.
There are some limited and very specific reasons when access to medical records can be refused, these will be discussed in detail with anyone to whom it applies.
